There’s a few storylines in this film:
*Tokyo-Karen is killed by Kayako after being pushed off the hospital roof. Eason was the one who pulled Karen out of the burning house in the 1st movie. He dies when Kayako attacks him in his photo development room. Aubrey, Karen’s sister, goes to Tokyo to investigate what happened to Karen, but Karen dies before she can explain. Aubrey goes to the house to try to stop the curse, but ends up getting killed by Kayako’s ghost husband thus replacing Kayako as the main ghost of the house. The ghost of Kayako we saw in the beginning terrorizing Allison was actually the ghost of Aubrey.
*Tokyo-After going into the cursed house, Miyuki is killed by Kayako after being pulled into a mirror and Vanessa is killed in a phone booth as Kayako wraps her hair around her. Allison is the only survivor of the 3.
*Chicago-The curse is somehow brought into the apartment building. Trish kills her husband, Bill (Christopher Cousins), by pouring hot oil over his head and whacking the pan over his head. She drowns in the bathtub (probably by Kayako), and also drowns her stepdaughter, Lacey. The hooded figure turns out to be Allison who had moved back to America thus bringing the curse and Kayako with her. Allison gets killed when Kayako pulls her into her hoodie. Jake (Matthew Knight) is the only survivor until Kayako grabs him when she comes out of Allison’s hood.
