The creatures in the cave are actually the same men from the beginning of the film who disappeared 30 years ago and got infected by a mysterious virus. Tyler, Top, and Kathryn (Lena Headey) are the only survivors. Kathryn reveals to Tyler she got infected before disappearing into the crowd.
